To Request Help

Please read the following carefully and fill out the form at the bottom of the page.

We believe that animals are family members and that families should stay together. If you or someone you know believes that a companion animal can no longer remain at home due to a financial or other issues, Family Forever Animal Foundation may be able to help.

We provide resources and education to pet guardians in order to prevent family animals from being surrendered to rescues and shelters. Each request for help will be evaluated. All assistance is at the discretion of FFAF.

Note that Family Forever Animal Foundation is a small, local, non-profit dependent on donations. If we cannot help, we will do our best to refer our requestors to other organizations and resources.
Please read and acknowledge the following before requesting assistance with Family Forever Animal Foundation.

  • To qualify:
  • The animal must be an “owned” pet. We are unable to help strays or community cats, fostered animals, independent rescuers, or 501(c)3s.
  • You must be a resident of one of the following counties: Brevard, Lake, Osceola, Orange, Polk, Seminole, Volusia 
  • To receive services, the animal must be spayed/neutered or requestor must agree to allow spay/neuter (medical exceptions allowed).
  • You must provide proof of the following to qualify for financial assistance:
    • Application and decline by Care Credit*
    • Approval for Care Credit in amount less than a cost of care estimate (owner is responsible for Care Credit approval amount)*
      *(if eligible to apply)

AND proof of one of the following

  • Qualification for public assistance (e.g. SNAP/WIC, SSI, Earned Income Tax Credit eligibility, utilities assistance, etc.)
  • Unemployment
  • Annual household income of <$50,000

Requestors must:

  • Make appointments and communicate appointment details to FFAF (instructions will be provided)
  • If a non-preferred veterinary practice is selected, requestor must ensure they accept phone or electronic payments 
  • Provide a photograph of the pet for use on social media/website
  • Provide updates to Family Forever Animal Foundation after care is provided

All requests for assistance are assessed on a case-by-case basis

  • Assistance is based on available funds and likelihood of funds making a difference in the outcome of the case.
  • Only secured funds are spent (no debt)
  • Preferred veterinarians will be recommended for treatment
  • Payments are made to veterinarians, food/supply companies directly. No funds are paid to requestors.
